Looking Forward: Examining Long-Term Services and Supports Integration and Opportunities to Strengthen Coordination Under CalAIM
In partnership with the Center for Health Care Strategies (CHCS) and Chapman Consulting, the California Health Care Foundation (CHCF) invites interested Medi-Cal managed care plans (MCPs), long-term services and supports (LTSS) partners, and other committed California stakeholders to a focused webinar discussion that builds on CHCF’s recently released issue brief, Meeting the Moment: Strengthening Managed Care’s Capacity to Serve California’s Seniors and Persons with Disabilities. This webinar will highlight forward-thinking lessons from key LTSS partners and Medi-Cal MCPs as they prepare to advance several reforms intended to improve care for seniors and persons with disabilities (SPDs), including those outlined in its California Advancing and Innovating Medi-Cal (CalAIM) initiative.

Panelists will discuss experiences developing partnerships under the Coordinated Care Initiative and will describe lessons around: (1) building and strengthening MCP and LTSS partnerships; (2) addressing barriers and leveraging opportunities to expand access to home- and community-based services; and (3) elevating strategies to support data sharing and improve care coordination. The target audience for this webinar includes MCPs, LTSS providers, advocates, and other key MCP partners with an expected role in advancing future reforms.
